29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. The control of hazardous energy authorized employee training provided to machine operators was inadequate in that did it not address the recognition of hazardous energy sources and the means necessary for energy isolation and control that were required for the Threader machine during upset conditions.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Employees who perform servicing and maintenance activities on or around the Threader machine were not required to: (1) Know the type and magnitude of the electrical, pneumatic and stored energy, the hazards of these energies to be controlled, and the method or means to control the energy. (2) Physically locate and operate energy isolating devices in such a manner as to isolate the machine from the energy source. (3) Affix lockout devices to the energy isolating device (4) Prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine have been accomplished. Machine operators were exposed to a struck-by, caught inbetween hazard from the horizontal movement of the upper die bar on the Threader machine between the transfer table and upper die bar while an operator retrieved raised bars and bars on the ground.
