Located at 301 West Main St., Saint Clairsville, Ohio 43950: a. On or about December 5, 2019, employees were exposed to fall hazards while working from a supported scaffold that had some of the legs and uprights that were not on adequate firm foundation. One of the legs and uprights did not have the adjustable leveling jack on a base plate and had settled into the ground. One of the legs and uprights was on a base plate but was displaced and not on a mud sill. b. On or about December 12, 2019, and at times prior, employees were exposed to fall hazards while working from a supported scaffold that had some of the legs and uprights that were not on adequate firm foundation. Access requirements for employees erecting or dismantling supported scaffolds are specifically addressed in paragraph (e)(9) of this section. 29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. Located at 301 West Main St., Saint Clairsville, Ohio 43950: a. On or about January 2, 2020, employees were exposed to fall hazards while accessing a supported scaffold platform that was approximately 14 feet - 6 inches above a point of access without an acceptable point of access.
