29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  Located at 301 West Main St., Saint Clairsville, Ohio 43950:  a. On or about December 5, 2019, employees were exposed to fall hazards while working from a supported scaffold that had some of the legs and uprights that were not on adequate firm foundation. One of the legs and uprights did not have the adjustable leveling jack on a base plate and had settled into the ground. One of the legs and uprights was on a base plate but was displaced and not on a mud sill.  b. On or about December 12, 2019, and at times prior, employees were exposed to fall hazards while working from a supported scaffold that had some of the legs and uprights that were not on adequate firm foundation. Located at 301 West Main St., Saint Clairsville, Ohio 43950:  a. On or about December 5, 2019, employees were exposed to fall hazards in that the footings on the scaffold in use were not level or sound and showed indications of settling and displacement. Some of the supported scaffold leveling jacks were on base plates that were displaced on top of mudsills and some were settled into the ground or unlevel.  b. On or about January 2, 2020, and at times prior, employees were exposed to fall hazards while working from a scaffold without being effectively trained on the proper use of the scaffolds. Employees were accessing the scaffold by climbing the side frames of the scaffold and climbing off the scaffold platform to access the roof.
