29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At the residential construction site located at 10002 North Revere Court, Kansas City, Missouri, 64154, Employees were exposed to struck by hazards. Employees were observed installing wood roof decking and materials using a pneumatic nail gun without eye/face protection.

29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times:  At the residential construction site located at 10002 North Revere Court, Kansas City, Missouri, 64154, employees were observed burning scrap wood in a burn barrel located adjacent to the residential construction. Employees were not protected from fire hazards by the use of fire extinguishers, which were not accessible to employees on the site.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  At the residential construction site located at 10002 North Revere Court, Kansas City, Missouri, 64154, employees were exposed to electrical shock and burn hazards. An electrical extension cord which was being utilized to power an electric Rolair Systems air compressor was frayed and had been wrapped with electrical tape.

The employee was observed using an A-frame ladder to install a window. The A-frame ladder was folded closed and was leaning on the wall of the residential construction.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the residential construction site located at 10002 North Revere Court, Kansas City, Missouri, 64154, employees were exposed to fall hazards. Employees were observed installing wood roof decking and other materials while working at elevation. Employees were exposed to fall hazards of between 11 feet 2 inches to 20 feet 5 inches to the ground below without use of fall protection.     Floriberto Cruz-Gonzalez dba Simon Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1406850, citation number 1, item number 1, and was affirmed as a final order on 07/24/2019, with respect to a workplace located at 24th & Campbell 2425, Kansas City, Missouri 64108.  Floriberto Cruz-Gonzalez dba Simon Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1082589, citation number 1, item number 1, and was affirmed as a final order on 08/26/2015, with respect to a workplace located at 16124 Earnshaw Street, Overland Park, Kansas 66221.
