29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On or before December 30, 2019, located on the east side of Tyler Technologies, Inc. facility, employees were exposed to 7200V line overhead power lines while operating a telescopic boom lift as there were no frequent and/or regular inspections of the job-site performed by a competent person such as guarding, isolating, or insulating, taken to prevent making contact with the power lines before the employees performing the work.

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.    On or before December 30, 2019, located on the east side of Tyler Technologies, Inc. facility, employees were installing metal siding onto an exterior commercial structure while operating a telescopic boom lift making contact with 7,200V line overhead power line that had not been deenergized and/or other protective measures such as guarding, isolating, or insulating, taken to prevent making contact with the power lines before the employees performing the work.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    a) The nature of any electrical hazards, fall hazards and falling object hazards in the work area;     b) The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used;      c) The proper use of the scaffold, and the proper handling of materials on the scaffold;       d) The maximum intended load and the load-carrying capacities of the scaffolds used; and       e) Any other pertinent requirements of this subpart.    On or before December 30, 2019, located on the east side of Tyler Technologies, Inc. facility, employees were installing metal siding onto an exterior commercial structure while operating a telescopic boom lift within 10 feet of a 7200V line overhead power line.
