29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place, when an employee (s) was required to remove or bypass a guard or other safety device:    On or about December 27, 2019 at the worksite located at 1750 Jesse Jewell Parkway in Gainesville, Georgia an employee suffered an amputation of the right lower arm, when the employer failed to ensure that lock out/tagout procedures were applied when the guard for the auger on the dust collection system was removed to facilitate cleaning operations.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
