On or about 01/07/2020, at the jobsite at 1900 Lafayette Ave., St Louis, MO, employee using battery operated Milwaukee Impact Driver, Model # J57AD193513186, to install a screw for a face board was not wearing safety glasses.

29 CFR 1926.501(b)(15): "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels were not be protected from falling by a guardrail system, safety net system, or personal fall arrest system. On or about 1/07/2020 at the job site at 1900 Lafayette Ave, St. Louis, MO, employees were installing forms for cement pour for steps. One of employees was observed standing inside of the bucket of a Bobcat Compact Skid Steer Loader in an attempt to install screws for the face board of the forms, exposing him to a fall hazard.
