29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not determined before opening the excavation: a) Jobsite: On January 7, 2020 and at times prior thereto the employer did not take the precaution needed to positively identify the location of utilities including a 2 inch electrical conduit and an existing 3 inch electrical conduit. The energized 3 inch electrical conduit (120/208y, 3 phase) was struck while performing trenching work to install a storm drain pipe. Note) Certification of corrective action is required for this item.
