29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Building 6 - On or about January 2, 2020 and at times prior thereto, employees were exposed to falls while framing the 3rd story floor not connected to an anchor point and no guardrails installed.  b) Building 6 - On or about January 15, 2020 and at times prior thereto, an employee was exposed to a fall hazard of 10' while bracing trusses above the 3rd floor without being connected to an anchor point.  c) Building 6 - On or about January 2, 2020 and at times prior thereto, employees were exposed to falls through stairwell openings that were not protected by guardrails.   On January 15, 2020 an employee was hospitalized with multiple injuries following a 10' fall from the trusses to the third floor below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) Building 6 - On or about January 2, 2020 and at times prior thereto, employees that were exposed to fall hazards had not been adequately trained by the employer in fall protection hazards.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:   a)  Building 6 - On or about January 2, 2020 and at times prior thereto, employees were exposed to falls while using the stairwell to the second floor which did not have a handrail.
