(a) On or about January 8, 2020, at the south side of the jobsite, employees utilized circular saws that had safety devices bypassed when in use. Employees placed wooden wedges in the guards of the saw to hold the guards back.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage    a) On or about January 8, 2020, at the south side of the jobsite, employees utilized several portable extension cords to power devices such as Kobalt circular saws, air compressors, and DeWalt reciprocating saw throughout the jobsite that had strain relief damage to the plugs, chafed shielding, and ground plugs missing.
