On or about January 8, 2020, at the above addressed jobsite, employees performing residential carpentry operations and roofing operations on a townhouse, twelve (12) feet eight (8) inches and twenty (20) feet seven (7) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about January 8, 2020, at the above addressed jobsite, an employee performing residential carpentry operations on a townhouse, nine (9) feet seven (7) inches for the basement access, and nine (9) feet (2) inches above the floor for the balcony, were not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  a.  On or about January 8, 2020, at the above addressed jobsite, an employee conducting carpentry operations on a roof was improperly tied off by wrapping his lanyard around a roof truss and attaching the snaphook to the lanyard strap, thereby exposing the employee to fall hazards of approximately twelve (12) feet eight (8) inches.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:  a.  On or about January 8, 2020, at the above addressed jobsite, an employee conducting carpentry sheeting operations on a roof was improperly tied off in that, the anchorage point for the personal fall arrest system (PFAS) was not capable of supporting at least 5000 pounds. The lanyard was wrapped around a roof truss that was not completely tied into the building, thereby exposing the employee to fall hazards of approximately twelve (12) feet eight (8) inches.
