29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program for all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: 1) On January 8, 2020, an employee operating a bandsaw and a thread rolling machine was exposed to noise at a dose of 76.8 %, or approximately 88.1 dBA, as an 8 hour time weighted average. The employer had not provided this employee, nor other employees in the production department, with a hearing test. 2) 1) On January 8, 2020, an employee operating a bandsaw and a thread rolling machine was exposed to noise at a dose of 70.7 %, or approximately 87.5 dBA, as an 8 hour time weighted average. The employer had not provided this employee, nor other employees in the production department, with a hearing test.

29 CFR 1910.95(k)(1): The employer did not train each employee who was exposed to noise at or above an 8 hour time weighted average of 85 dBA in accordance with the requirements of this section: 1) On January 8, 2020, employees were exposed to noise at a level that exceeded the 85 dBA. The employer had not trained these employees, nor other employees in the production department, on the information contained in paragraph (3) of this section, including: a) the effects of noise on hearing; b) the purpose of hearing protectors, the advantages, disadvantages, and attenuation of the various types, and instructions on selection, fitting, use, and care; c) the purpose of audiometric testing, and an explanation of the test procedures.

Employees work with chemicals that are flammable and toxic such as spray paint containing xylene, toluene and/or ethylbenzene, as well as metal working fluids containing mineral oil, an eye and skin irritant.
