29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) 80 Commerce Way, Totowa, NJ/Roof: Employee performing roofing work was exposed to a 20 foot fall hazard. On 12/24/2019, An employee fell approximately 20 feet while engaged in roofing work.   Quality Home Contractors Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1430477, citation number 1, item number 1 and was affirmed as a final order on 11/18/2019, with respect to a workplace located at  400 Colfax Ave Clifton, NJ 07013.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours.  a) 80 Commerce Way, Totowa, NJ/Roof: The employer did not report the in-patient hospitalization of an employee to OSHA within twenty-four (24) hours. On 12/28/2019 OSHA was notified of a work-related injury to an employee on 12/24/2019 requiring in in-patient hospitalization.
