29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:    a) On or about January 9, 2020 on the north side of the site, employees were observed working from 2x4 scaffold at a height of 6 feet two inches to install a vapor barrier on a new construction house, that scaffolding had a walking width of 6 inches.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    a) On or about January 9, 2020, on the north side of the jobsite, employees utilized 2x4 scaffolding to install a vapor barrier on a now construction home that supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) On or about January 9, 2020, on the north side of the worksite, employees were utilizing 2x4 scaffolding at a height of six feet two inches that did not have means of access or egress.
