29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.  a- Building Exterior: An employee accessing a tubular welded frame scaffold was exposed to electrocution hazards as an energized secondary power line was positioned within inches of the scaffold which had not been de-energized. Condition noted on or about 11/25/2019.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a- Residential Roof:  An employee engaged in roofing activities on a 9 on 12 pitched roof was exposed to fall hazards of 20ft to the concrete below as a means of fall protection was worn but not secured via an anchor point. Condition noted on or about 11/25/2019.
