29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about January 9, 2020, and times prior thereto:    a. In the Back Shop Area, where employees were exposed to struck-by and caught-by hazards when operating a Okuma LB4000 EX II CNC lathe #LBE-041 with the interlocks by-passed.     b. In the Back Shop Area, where employees were exposed to struck-by and caught-by hazards when operating a Okuma LB4000 EX II CNC lathe #LBE-040 with the interlocks by-passed.     c. In the Back Shop Area, where employees were exposed to struck-by and caught-by hazards when operating a Okuma LB4000 EX II CNC lathe #LBE-039 with the interlocks by-passed.     d. 29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:  On or about January 9, 2020, and at times prior thereto, on the outside rear of the Back Shop Building, oxygen cylinders were stored without being separated from fuel-gas cylinders a minimum distance of 20 feet.
