The certifications were due to OSHA by March 3, 2021 and were not provided.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At a worksite located at 1827 Buchanan Street, North Kansas City, Missouri:  Employees were exposed to fall hazards.  Employees were climbing on the railings of the scissor lift to gain access to the roof.  They employees had greater than a 10 feet fall to the concrete sidewalk.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  At a worksite located at 1827 Buchanan Street, North Kansas City, Missouri:  Employees were exposed to fall hazards.  Employees were exposed to a scissor lift which did not have a top rail chain at the entrance end of the scissor lift.  Employees were being lifted to the top of the building with a scissor lift that was missing the top chain at the entrance end of the scissor lift.

At a worksite located at 1827 Buchanan Street, North Kansas City, Missouri: Employees were exposed to fall hazards.  Employees were observed conducting plumbing work on the roof and working at height.  Employees were exposed to a fall hazard of approximately 15 feet without the use of fall protection.
