29 CFR 1926.104(d): Safety belt lanyard(s) were not a minimum of l/2 inch nylon, or equivalent: At worksite located at 6th and East St, Parkville, MO -- Two employees were in a Genie S-65 aerial lift. One of the employees was using an extension cord as a means of a lanyard. This was not equivalent to the minimum 1/2 inch nylon required for a safety belt lanyard. This exposed the employee to a fall hazard of approximately 20 feet to the ground and to be catapulted from the aerial lift. This was documented on or about 01/07/2019.

29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: At work site located at 6th and East St, Parkville MO -- Two employees were in a Genie S-65 aerial lift and did not have a safety harness with lanyard attached to the boom or basket. This exposed employees to fall hazards of approximately 20 feet to the ground and to be catapulted from the aerial lift. This was documented on or about 01/07/2019.
