29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices (RAGAGEP).  On or about January 7, 2020, and at times prior thereto, employees were exposed to struck-by and burns hazards when the employer at Calcasieu Refining Company Crude #2 Unit in Lake Charles, Louisiana did not document that equipment, including the pressure relief systems complied with the employer's chosen RAGAGEP such as but not limited to ASME Boiler and Pressure Vessel Code, Section VIII Division 1, Part UG-135(d) when there were block valves in the relief paths and they were not positively secured in the opened position. Instances include but not limited to;  a) E-208 (Heat Exchanger) PSV. b) V-207 (Prefrac Accumulator) PSV.

29 CFR 1910.119(f)(1)(iv): Safety systems and their functions. The employer did not implement operating procedures for protecting safety systems and their functions.  On or about January 8, 2020, and at times prior thereto, employees were exposed to struck-by and burns hazards when the employer at Calcasieu Refining Company Crude #2 Unit in Lake Charles, Louisiana did not implement the Car Seal Procedure to protect the relief path to the following PSVs to ensure they function when needed. Instances include but not limited to;  a) E-208 (Heat Exchanger) PSV. b) V-207 (Prefrac Accumulator) PSV.

On or about January 7, 2020, and at times prior thereto, employees were exposed to struck-by and burn hazards when the employer at Calcasieu Refining Company #2 Crude Unit in Lake Charles, Louisiana did not ensure process equipment were inspected and maintained to ensure mechanical integrity. Instances include but not limited to;  a) 1"-UN-20111-A2-1-H2 piping on V-206 (Prefrac Tower - Crude #2 Unit).

29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the PHA team's findings and recommendations;  On or about January 7, 2020, and at times prior thereto, employees were exposed to struck-by and burns hazards when the employer at Calcasieu Refining Company in Lake Charles, Louisiana did not establish a system to promptly address the 2017 PHA team's findings and recommendations for Stabilizer Units #6 and #4. Instances include but not limited to;  a) The Facility did not verify the Flare Header outlet piping relieves into the vapor space of a system and is free draining. This is required for vapor relief scenarios per API 520.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:   a) On or about January 8, 2020, at Crude #2 Unit, Calcasieu Refining, Lake Charles, Louisiana, the employees utilized unlabeled and unidentified spray bottles containing a toxic and irritant chemical (Petro-Rite HD Rigwash Concentrate).
