29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about January 10, 2020, the employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed and the dates of the hazard assessments. The employer had performed PPE assessments for employees installing solar panel modules that addressed hazards which could affect employee's Eyes, Head/Neck/Face, Respiratory, Hearing, Hands/Arms, Torso, and Feet, but did not have the assessments signed and dated.
