29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. Located at 115 Enterprise Parkway, West Jefferson, Ohio: On or about January 04, 2020, employees were exposed to caught-in and nip point hazards while servicing the CRUZbelt conveyor system, M NO: M4201225 and M NO: M4201140, Tag: SH2002AD. The employer did not ensure that energy control application steps were implemented as the conveyor was not shut down or turned off to perform the servicing work of lubrication [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], and verification of isolation [(d)(6)], were not implemented to protect employees from the machine servicing hazards.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. Located at 115 Enterprise Parkway, West Jefferson, Ohio: On or about January 04, 2020, the employer did not report the employee's amputation to OSHA that resulted at the worksite location, within twenty-four hours. The employee had amputated his right index finger as a result of the caught-in and nip point hazard at the worksite.
