Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) On or about January 14, 2020 and at times prior thereto, employees were exposed to powered industrial truck (PIT) accidents, due to the employer modified the PIT caterpillar forklift (m/n GC25, s/n 4EM04700) without approval from the manufacturer.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a) On or about January 14, 2020 and at times prior thereto, employee(s) operate a caterpillar forklift truck (m/n GC25, s/n 4EM04700) daily during his work shift without inspecting it prior to use.
