29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance. The employer is failing to develop and utilize specific lockout tagout procedures for the Batchman power control panels that would ensure the employee safety associated with electrical hazards. This was most recently documented on January 14, 2020 at the Lyman-Richey facility located at 4765 S. 135th Street Omaha, NE 68137.

29 CFR 1910.1200(f)(6)(ii): Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. The employer is failing to label secondary containers in accordance with this subpart. This was most recently documented on January 14, 2020. Pump spray units were being used by employees that were filled with Liquid Heat with only black marker name of product on container.
