29 CFR 1910.23(d)(4): The side rails of through or sidestep ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: a) Line 1 Trench Conveyor Area in the Front End - On or about January 7, 2020, a fixed industrial ladder (South Ladder) located on the south side of the trench did not have side rails that extend 42 inches above the top of the access level or landing platform (the floor). Side rails of the South Ladder were just below floor level at approximately 49 inches high from the ground of the trench. The floor measured to be approximately 52 inches high from the ground of the trench.
