29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guiderail system and the walking-working surface: (a) At the yard storage building, on or about January 15, 2020 and at times prior to, the stairs (6 steps) used to access the building was missing midrails between the top edge of the guiderail system and the walking-working surface, thereby exposing employees to a fall hazard.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) In the facility yard, on or about January 21, 2020, employees who were required to wear tight-fitting respirators were permitted to have facial hair that came between the sealing surface of the facepiece and the face, thereby exposing employees to respiratory hazards.
