29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: a) Jobsite, 216 & 218 Bower Hill Road, Pittsburgh, PA: On or about January 15, 2020, two employees were installing shingles on a residential roof at heights of approximately 31 feet and were not utilizing fall protection. The employer told the csho that the employees know how to work at heights from their previous employers, but did not verify that the training the employees had received from their prior employers was adequate and was still being followed.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite, 216 & 218 Bower Hill Road, Pittsburgh, PA: On or about January 15, 2020, two employees were using a Werner 32 foot extension ladder to gain access to the roof they were working on and the ladder was only extended 24 inches above the landing surface.
