29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: A) North West exterior of the building, on or about January 13, 2020, wooden pallets, and loose debris were scattered on the ground, thereby exposing employees to a tripping hazards. B) The 6th and 7th floor, on or about January 13, 2020, plastic debris, and electrical cords were scattered in the hallway, thereby exposing employees to a tripping hazards.

29 CFR 1926.1052(b)(2): Skeleton metal stairs where permanent treads and/or landings were to be installed at a later date, were not fitted with secured temporary treads and landings long enough to cover the entire tread and/or landing area and foot traffic: A) North West section of building, on or about January 13, 2020, the stair pan treads for the #2 stairway were not filled in with concrete or other material, thereby exposing employees to a trip/fall hazard.
