29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On 1/14/2020, employees of Rosales General Contracting LLC were working on a residential building located at 15599 Hitchcock Road, Chesterfield, MO.  Employees were using a Bostitch coil roofing nail gun without the use of eye protection.

29 CFR 1926.300(b)(2): Moving Part(s) of equipment exposed to contact by employees or which otherwise created a greater hazard, were not guarded:  On 1/14/2020, employees of Rosales General Contracting LLC were re-roofing a residential building located at 15599 Hitchcock Road, Chesterfield, MO.  Employees were exposed to caught in-between hazards while operating a Grip Rite GR55G-10X air compressor.  The guard for the Honda starter motor was removed, while in use.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On 1/14/2020, employees of Rosales General Contracting LLC were re-roofing a residential building located at 15599 Hitchcock Road, Chesterfield, MO.  Employees were installing shingles approximately 11 feet above a lower level.  Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.

Training was not provided to employees in recognition of fall hazards and the procedures necessary to reduce or eliminate these hazards.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  On 1/14/2020, employees of Rosales General Contracting LLC were re-roofing a residential building located at 15599 Hitchcock Road, Chesterfield, MO.  Portable ladders were positioned around the residence, the side rails did not extended three feet beyond the upper landing surface or edge.  The ladders were being used by employees.  The fall distance was approximately 9-11 feet to the ground below.
