29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) In the wood-shop area, at the C.W.M.I. brand, model SC-18, #2 chop-saw: On or about 01/13/2020, the employer exposed employees to electrical shock and struck-by hazards, in that the employer developed and documented a procedure for the application of energy control (contained within the employer's hazardous energy control program) which was not utilized by employees while clearing jams to the saw's integrated dust control system.  b) In the wood-shop area, at the C.W.M.I. brand, model SC-18, #2 chop-saw: On or about 01/15/2020, and at times prior, the employer exposed employees to electrical shock and struck-by hazards, in that the employer developed and documented a procedure for the application of energy control (contained within the employer's hazardous energy control program) which was not utilized by employees who routinely opened the saw's lower door (a machine guard) to clean the interior components.  OR IN THE ALTENATIVE: 29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.
