29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) In the wood-shop area, at the C.W.M.I. brand, model SC-18, #2 chop-saw: On or about 01/13/2020, the employer exposed employees to electrical shock and struck-by hazards, in that the employer developed and documented a procedure for the application of energy control (contained within the employer's hazardous energy control program) which was not utilized by employees while clearing jams to the saw's integrated dust control system. b) In the wood-shop area, at the C.W.M.I. brand, model SC-18, #2 chop-saw: On or about 01/15/2020, and at times prior, the employer exposed employees to electrical shock and struck-by hazards, in that the employer developed and documented a procedure for the application of energy control (contained within the employer's hazardous energy control program) which was not utilized by employees who routinely opened the saw's lower door (a machine guard) to clean the interior components. OR IN THE ALTENATIVE: 29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) In the wood-shop area, at the C.W.M.I. brand, model SC-18, #2 chop-saw: On or about 01/13/2020, the employer exposed employees to electrical shock and struck-by hazards, in that Authorized employee training was not provided to employees who were directed to remove a machine guard (open a machine guard door) and place part of the employees body into an associated danger zone of a rotating saw blade while clearing jams to the saw's integrated dust control system. b) In the wood-shop area, at the C.W.M.I. brand, model SC-18, #2 chop-saw: On or about 01/15/2020, and at times prior, the employer exposed employees to electrical shock and struck-by hazards, in that Authorized employee training was not provided to employees who were directed to remove a machine guard (open a machine guard door) to clean the saw's interior components.
