29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined on the basis of arcing effects: (a) Jobsite, Electrical Room: On or about January 14, 2020, the electrical contractor did not ensure that a #6 ground wire was not able to come in contact with energized electrical parts on the inside of the Main Distribution Panel (MDP).

29 CFR 1926.404(f)(8)(ii): Conductor used for grounding fixed or movable equipment did not have capacity to conduct safely any fault current which may be imposed on it: (a) Jobsite, Electrical Room: On or about January 14, 2020, the #6 ground wire did not have the capacity to safely conduct any fault current which was imposed on it. The #6 copper ground wire melted when it came in contact with the energized bus bar.
