29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about January 14th, 2020, the employer did not ensure employees were in use of fall protection, exposing the employees to fall hazards greater than six (6) feet to the ground below.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about January 14th, 2020 the employer used an unauthorized scaffold platform, attached to a Genie Rough Terrain Forklift to elevate two employees on the roof of a building. This exposed the employees to a fall hazard.
