Throughout the Facility at 2033 Main St Athol, MA: On or about January 15, 2020 the employer had not provided an asbestos awareness training to housekeeping employees who perform daily sweeping and mopping of areas containing asbestos containing materials.

At 2033 Main Street Athol, MA: On or about 1/15/2020, the employer had not affixed signs or labels on or posted near asbestos 9"x9" floor tile and asbestos containing thermal system insulation (TSI) on piping above suspended ceiling tiles. Employees enter and work in these areas daily and housekeeping staff perform cleaning of floors on a daily basis.
