29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Western facing side of commercial building 6038 Old Pearsall Road: On or about January 14, 2020, and at times prior thereto, employees were exposed to struck by hazards while installing fascia board with a pneumatic nail gun without wearing eye protection.

29 CFR 1926.405(g)(1)(iii)(D): Flexible cords were attached to building surfaces:    a). Western facing side of commercial building 6038 Old Pearsall Road: On or about January 14, 2020 and at times prior thereto, employees were exposed to electrical hazards while using an extension cord that was attached to the rooftop area and was used as a power source for a Dewalt portable air compressor.    b). Western facing side of commercial building 6038 Old Pearsall Road: On or about January 14, 2020 and at times prior thereto, employees were exposed to electrical hazards while using an extension cord that was attached to the rooftop area and was used as a power source for a hand held circular skill saw.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    Western facing side of commercial building 6038 Old Pearsall Road: On or about January 14, 2020, and at times prior thereto, employees were exposed to fall hazards while using a Genie Telehandler that was supporting a plywood pallet to elevate personnel who were applying exterior fascia board without the use of fall protection.
