An employee fell approximately 19 feet when a joist beneath a metal roof panel broken under the employee's weight.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: (a) Jobsite, 281 Route 30, Imperial, PA: On or about January 15, 2020 an employee unhooked from a rope grab and began to walk across the roof to the ladder access when a roof joist below a metal panel broke free and bent causing the employee to fall approximately 19 feet to the interior of the structure. had not been trained by a competent person in the correct procedures for erecting and maintaining fall protection systems being used. An employee unhooked from a rope grab and began to walk across the roof to the ladder access when a roof joist below a metal panel broke free and bent causing the employee to fall approximately 19 feet to the interior of the structure. Employee did not receive sufficient, consistent fall protection training or instruction by a competent person on remaining tied off when traversing to and from an access point and at all times while working.
