29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  On or about January 16, 2020, throughout the Pallet Shop, employees were exposed to walking-working surfaces that were not kept in a clean, orderly, and sanitary condition due to the disorderly placement of pallets, wood scrap, and piles of saw dust.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a. On or about December 16, 2019, and at times prior to, employees performed maintenance on machinery to include the Pallet Dawg machine.  The employer had not established a program consisting of energy control procedures, employee training and inspections to ensure the startup or release of stored energy could not occur while servicing or maintenance.    b. On or about January 16, 2020, and at times prior to, employees performed maintenance on machinery to include the Horizontal Band Saw machine. The employer had not established a program consisting of energy control procedures, employee training and inspections to ensure the startup or release of stored energy could not occur while servicing or maintenance.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  On or about January 16, 2020, and at times prior, the employer did not ensure that all stored materials were stacked and blocked so that materials were stabled and secured against sliding and collapse to prevent employee injury.  Throughout the facility employees were exposed to struck by hazards while working near wooden pallets stored in tiers without any means to secure the material against sliding and collapse.

The propane fuel tank was not properly secured to the Hyundai forklift, (Model 30L-7A; Serial # HHIHHF09J130000148) .
