29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  The employer is failing to protect employees from fire and electrical hazards. This was most recently documented on 01/16/2020 at the site location of 3753 SE Convenience Blvd, Ankeny, Iowa. The employer failed to protect employees from electrical hazards in the following, but not limited to:    a) Located in employee number 1s work area, a relocatable power tap was connected in series (daisy chained) to a second relocatable power tap used to power a printer and fan.   b) Located in employee number 2s work area, an extension cord was plugged into a relocatable power tap in order to plug in a cell phone charger.   c)  Located in the storage cabinet near the south exit, a relocatable power tap was connected to an extension cord used to power an iPad charging bank.  d) Located in employee number 3s work area, a Honeywell HHF 360B electric space heater was plugged into a relocatable power tap.  e)  Located in employee number 4s work area, a Comfort Zone electric space heater was plugged into a relocatable power tap.

29 CFR 1910.304(g)(6)(iii): Frames of electric ranges, wall-mounted ovens, counter-mounted cooking units, clothes dryers, and metal outlet or junction boxes that are part of the circuit for these appliances shall be grounded.  The employer is failing to protect employees from fire and electrical hazards while utilizing a Cuisinart toaster. This was most recently documented on 01/16/2020 at 3753 SE Convenience Blvd, Ankeny, Iowa. The metal frame of the Cuisinart model RBT-385 toaster indicated a presence of A/C voltage.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.  The employer is failing to protect employees from electrical shock, burn, and fire hazards associated with the improper use of flexible cords and cables. This was most recently documented on 01/16/2020 at the site location of 3753 SE Convenience Blvd, Ankeny, Iowa. The flexible cord of a relocatable power tap was directed through an unprotected metal opening in the cabinet to power an iPad charging station.

29 CFR 1910.334(a)(3)(iii): Adapters which interrupt the continuity of the equipment grounding connection may not be used.  The employer is failing to protect employees from fire and electrical hazards while utilizing flexible cords and cables. This was most recently documented on 01/16/2020 at the site location of 3753 SE Convenience Blvd, Ankeny, Iowa. An employee was utilizing an ungrounded plug adapter to power a computer monitor with a 3 prong plug (plug with a grounding pin) into a 2 prong extension cord.
