29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms and walking-working surfaces were not kept clean, orderly and in sanitary condition.    (a) On or about January 16, 2020, and at times prior, Firestone Complete Auto Care did not ensure that passageways and walking-working surfaces were maintained in a sanitary condition. Employees were exposed to slip and trip hazards due to lack of housekeeping in the main shop. Employer had tires, tools, and equipment scattered on the passageways and walking-working surfaces. 29 CFR 1910.141(d)(1): Washing facilities were not maintained in a sanitary condition:  (a) On or about January 16, 2020, and at times prior, Firestone Complete Auto Care did not ensure that washing facilities were maintained in a sanitary condition. Employees were exposed to health hazards in that the washing/toilet facility for the employees was not kept clean and a sanitary condition. Significant amounts of waste, debris, and extensive stains were observed on the walls, floors, surfaces and water fixtures. 29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    (a) On or about January 16, 2020, and at times prior, Firestone Complete Auto Care did not ensure that exit routes were kept free and unobstructed. Employees were exposed to fire and entrapment hazards in that the exit at the far end of the main shop was blocked by tires, and equipment.      Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Firestone Complete Auto Care was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1194488, citation number 2, item number 2b and was affirmed as a final order on February 13, 2017, with respect to a workplace located at 18701 E. Hampden Ave. Aurora, CO 80013.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    (a) On or about January 16, 2020, and at times prior, Firestone Complete Auto Care did not ensure that sufficient access and working space was provided and maintained about electric equipment operating at less than 600 volts. Employees were exposed to electrical hazards in that access to electrical panels in the main shop was blocked by tools, equipment, and materials.      Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). 29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:  (a) On or about January 16, 2020, and at times prior, Firestone Complete Auto Care did not ensure that each container of hazardous chemicals was labeled, tagged or marked with specific information regarding the physical and health hazards of each hazardous chemical. Employees were exposed to contact with chemical hazards in that the employer did not properly label containers with hazardous chemicals, including containers with windshield washer fluid and automobile antifreeze/coolant. These substances have chemical components such as methyl alcohol, and ethylene glycol, which can cause serious irritation to eyes/skin. These chemical components are also toxic after ingestion, inhalation, and contact with the skin.
