A gangway or ramp used to access a barge lacked guardrails and/or a safety net erected under for protecting employees from falling into water, exposing employees to the risk of drowning.

29 CFR 1926.605(d)(3): Employees working or walking on the unguarded decks of barges were not protected with U.S. Coast Guard-approved work vests or buoyant vests: On or about 1/16/2020, at ASAP Installations LLC worksite located at 915 Cordova Road, Fort Lauderdale, FL 33326. Employees were exposed to a drowning hazard while working over a barge without wearing a personal flotation device. In the alternative: 29 CFR 1926.106(a): Employees working over or near water, where the danger of drowning exists, shall be provided with U.S. Coast Guard-approved life jacket or buoyant work vests. On or about 1/16/2020, at ASAP Installations LLC worksite located at 915 Cordova Road, Fort Lauderdale, FL 33326. Employees working over and near water were not wearing a personal flotation device, exposing them to a drowning hazard.
