At the Primary Clarifier walkway area, located at 7800 Delaroche Drive South in Jacksonville, Florida: On or about January 16, 2020, the employer exposed employees to slip, trip, and fall hazards, in that the walkway had bolts protruding from the concrete surface where employees performed spraying operations on the walkway.

At the walkway on top of the Digester Building, located at 7800 Delaroche Drive South in Jacksonville, Florida: On or about January 16, 2020, the employer exposed employees to fall hazards, in that the existing walkway guardrail system did not utilized a mid-rail when employees decanted the digester. b. At the Primary Clarifier area, located at 7800 Delaroche Drive South in Jacksonville, Florida: On or about January 16, 2020, the employer exposed employees to raw sewage and fall hazards, in that covers and/or guard-rails were not utilized by employees while working in the area. c. At the Wasting Pit area, located at 7800 Delaroche Drive South in Jacksonville, Florida: On or about January 16, 2020, the employer exposed employees to raw sewage and fall hazards, in that covers and/or guard-rails were not utilized by employees while working in the area. d. At the Air Bay area, located at 7800 Delaroche Drive South in Jacksonville, Florida: On or about January 16, 2020, the employer exposed employees to raw sewage and fall hazards, in that covers and/or guard-rails were not utilized by employees while working in the area.
