29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition.    On or about January 16, 2020, and at times prior thereto:    a. In the Building 1 Threading Area, where employees  were exposed to slip, trip, and fall hazards when performing pipe threading work without the walking-working surfaces kept in a clean, orderly, and sanitary condition.    b. In the Building 2 Threading Area, where employees  were exposed to slip, trip, and fall hazards when performing pipe threading work without the walking-working surfaces kept in a clean, orderly, and sanitary condition.    c. In the Building 2 Cutting Area, where employees  were exposed to slip, trip, and fall hazards when performing pipe cutting work without the walking-working surfaces kept in a clean, orderly, and sanitary condition.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    On or about January 16, 2020, and at times prior thereto, in the Cutting Area where employees were exposed to an electrical shock hazard when operating a 230-VAC Thermal Dynamics plasma welder plugged into shop-made electrical cord with a metal junction box attached at the end. The junction box was not used as listed and labeled.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about January 16, 2020, and at time prior thereto:  a. In the middle section of the Cutting Area, where employees operated a Thermal Dynamics plasma cutter, Cutmaster 102 with an electrical cord was not connected to the plug so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.   b. On the west wall of the Cutting Area, where employees operated an Ingersoll Rand air compressor with an electrical cord was not secured to the circuit breaker box so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.
