29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:    (a)Black Hills Industries dba Black Hills Roofing at 14 Wolf Creek Road, Pine Ridge, SD 57770: On or about and at times prior to January 14, 2020, the controlling employer did not ensure subcontractor employees were protected from fall hazards in that, the warning line system being used for fall protection was not fully erected around the work area on a commercial roof  under construction. Subcontractor employees were working on the roof of the Ogalala Lakota County CTE High School with an inadequate warning line system that did not encompass the work area. An employee was working in an area that was outside of where the warning line terminated in close proximity to the edge of the roof exposed to a fall hazard. This condition exposed employees to a fall hazard of approximately 16 feet.
