29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  a)     Paducah Barge located at 1024 Port Authority Road, Eddyville, KY: On or about January 14, 2020, a flexible extension cord used to provide power to hand tools, (electric grinders, electric drills and electric saws), was not grounded exposing workers to potential electric shock.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  a)     Paducah Barge located at 1024 Port Authority Road, Eddyville, KY: On or about January 14, 2020, the outer insulation of an extension cord was separated from the plug exposing workers to potential electrical shock.  b)     Paducah Barge located at 1024 Port Authority Road, Eddyville, KY: On or about January 14, 2020, flexible cords used to power the Plasma Cutter/Air Compressor box were not protected where they enter the electrical panel box.  c)     Paducah Barge located at 1024 Port Authority Road, Eddyville, KY: On or about January 14, 2020, a cable leading into the main power panel the provides power to the site was not protected where it entered the panel.
