29 CFR 1915.73(d): Guardrails were not provided at locations where employees are exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which present a hazard of falling more than 4 feet (1.22 m) or into the water, except as specified in paragraph (b)(2) of this section. a) At captive barge Mississippi, upstream, on or about 16 Jan 2020, the employer exposed employee to fall hazards in that top rail along the river side was not utilized to prevent employees from falling 4 feet or more. b) At captive barge Missouri, downstream, on or about 16 Jan 2020, the employer exposed employee to fall hazards in that top rail along the river side was not utilized to prevent employees from falling 4 feet or more. c) At captive barge White Tail 3, downstream, on or about 16 Jan 2020, the employer exposed employee to fall hazards in that top rail along the river side was not utilized to prevent employees from falling 4 feet or more. d) At captive barge Missouri, upstream, on or about 16 Jan 2020, the employer exposed employee to fall hazards in that top rail along the shore side was not utilized to prevent employees from falling 4 feet or more.
