29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4): (a) On or about December 19, 2019, the employer did not ensure that employees working with Milwaukee 4-1/2 inch Small Angle Grinders were protected from caught-in and amputation hazards as the employees were operating the grinders with the guards removed while conducting steel grinding operations. 29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: (a) On or about December 6, 2019, the employer did not ensure that employees conducting steel grinding operations in a make-shift room were informed about the location of fire extinguishers. A small fire broke out when grinding sparks were contacted by the box fans used in the room and the employees were not sure where the fire extinguishers were located.
