29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4):    (a) On or about December 19, 2019, the employer did not ensure that employees working with Milwaukee 4-1/2 inch Small Angle Grinders were protected from caught-in and amputation hazards as the employees were operating the grinders with the guards removed while conducting steel grinding operations.      Abatement note: Abatement certification is required for this item (See Certification of Corrective Action Worksheet).

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  (a) On or about December 6, 2019, the employer did not ensure that employees conducting steel grinding operations in a make-shift room were informed about the location of fire extinguishers. A small fire broke out when grinding sparks were contacted by the box fans used in the room and the employees were not sure where the fire extinguishers were located.  Abatement note: Abatement certification is required for this item (See Certification of Corrective Action Worksheet).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) On or about December 19, 2019, the employer did not ensure that employees were protected from electrical hazards in that relocatable power taps or "power strips" were not used in accordance to their listing and labeling instructions in that they were not directly connected to a permanent receptacle, and were connected to other extension cords.
