29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about January 15, 2020, and at times prior thereto, workers were installing siding on the apartment buildings under construction up to approximately 24 feet high from pump jack scaffolds that did not have guardrails installed on the ends of platforms, exposing employees to a fall hazard.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: On or about January 15, 2020, and at times prior thereto, the second story balconies of the apartment building under construction, up to approximately 11 feet high, did not have guardrails or other fall protection, exposing workers to a fall hazard.
