The self retracting life line used by the employee had visible wear, damage, and deterioration present on the energy absorber and its cover.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: On or about December 19, 2019, the employer failed to ensure that each employee exposed to a fall of approximately 15 feet while performing welding activities on the edge of a precast concrete structure had the understanding and skill needed to properly inspect and to correctly use and operate their personal fall arrest systems.
