29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a) New office construction located at 11380 State Bridge Road, Alpharetta, GA: Roof: On or about 01/10/2020, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in siding installation activities.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) New office construction located at 11380 State Bridge Road, Alpharetta, GA: Rear side cut bench: On or about 01/10/2020, and at times prior, the employer exposed employees to severe laceration/amputation hazards while operating circular saws, in that the lower guard was shimmed in the upward position and not retracting to guard the blade as it was unguarded after each cut was made.

(a) New office construction located at 11380 State Bridge Road, Alpharetta, GA: Roof: On or about 01/10/2020, and at times prior, the employer exposed employees to fall hazards of 19-feet, 8-inches to the lower level, in that a fall protection system was not used while engaged in siding activities on a dormer located on a 7/12 pitched roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) New office construction located at 11380 State Bridge Road, Alpharetta, GA: Roof: On or about 01/10/2020, and at times prior, the employer exposed employees to fall hazards of 19-feet, 8-inches to the lower level, in that the ladder provided was not extended at least 3-feet past the roof landing to allow employees safe access onto the roof and off the roof to the lower level.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) New office construction located at 11380 State Bridge Road, Alpharetta, GA: Rear side of building: On or about 01/10/2020, and at times prior, the employer failed to inspect all of the electrical cords prior to the start of the work shift, in that a 50-ft. extension/flexible cord used to power a Makita (Model 5007F) 7-1/4-inch Circular Saw had a torn outer casing, and damaged strain relief and was not repaired or removed from service.
