On or about January 21, 2020 Subcontractor Henry Contracting, Inc employees were observed installing wood roof trusses on a 2 story, 6:12 pitch commercial structure were not protected by guardrail systems, safety net systems, or personal fall arrest systems, exposing employees to 12-foot fall hazard.
