29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) On or about January 8, 2020, the work area was disorganized and unkept, exposing employees to punctures from protruding nails and slips, trips and fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about January 8, 2020, employees were exposed to injuries from falls, while conducting residential construction activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.
