29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:  a) On or about December 4, 2019 energy control procedures and/or equivalent protection was not required to be utilized by employees when cleaning activities such as but not limited to, scraping build-up from rotating rollers and/or idlers on the Rigid Coater 11.     b) Energy control procedures and/or equivalent protections was not required to be utilized by employees when performing setup activities, such as but not limited to, threading the melt curtain through rollers and the lam drum, and/or adjusting the bolts on the melt curtain die, while the web was moving through the coater line.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    On or about December 4, 2019, employees were exposed to machine hazards associated with moving parts when they engaged in cleaning activities such as but not limited to scraping rollers and idler rollers activities on the Rigid Coater 11. The employer did not ensure that employees implemented energy control application steps prior to allowing them to clean. The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.     As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

1910.212(a)(3)(ii) : The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   The employer did not ensure that a rotating idler roller on Coater 11 was adequately guarded to protect employees when manually removing residue from the surface of the idler roller during production of coated sheets.
