29 CFR 1910.28(b)(7): 1910.28(b)(7): The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling: (a) Stone Building Products, LLC dba Signature Stone at 211 30th St. Greeley, CO 80631: On or before 1/22/2020 the employer did not ensure that the opening on the concrete mixer [Wiggert + Co GmbH Model: Mobilmat 45 - Mo45/4-PCS-W; Serial # 58083] discharge upper hatch was adequately guarded and that its interlock was serviceable. The hatch is opened during cleaning operations as well as other times. The interlock was inoperable. This condition exposed employees to fall and crushing hazards.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: (a) Stone Building Products, LLC dba Signature Stone at 211 30th St. Greeley, CO 80631: On or before 1/22/2020 the employer did not protect employees from injury in that the fixed metal stairs leading to the upper work platform of the concrete mixer was equipped with a hand rail on its lower six steps. This condition exposed employees to slip, trip, and fall hazards.
